 Introduction Anemia of Chronic Disease (ACD), also known as Anemia of Inflammation (AI), is the second most common type of anemia after iron deficiency anemia. It develops as a complication of chronic inflammatory disorders rather than a primary hematologic disease. Patients with chronic infections, autoimmune diseases, malignancies, or chronic kidney disease are at increased risk of developing this condition. Unlike iron deficiency anemia, the body usually contains sufficient or even increased iron stores. However, persistent inflammation alters normal iron metabolism, preventing iron from being efficiently utilized for hemoglobin synthesis and red blood cell production. Although the body has adequate or even excessive iron stores, the iron cannot be effectively incorporated into hemoglobin. Instead, it accumulates within the mitochondria of developing erythroblasts, forming ring sideroblasts, the characteristic hallmark of the disease. Sideroblastic anemia may be either congenital or acquired. The condition most commonly develops because of vitamin B12 (cobalamin) deficiency or folate (vitamin B9) deficiency, both of which are essential for normal cell division and red blood cell maturation. Although red blood cells are primarily affected, severe cases may also reduce white blood cells and platelets, leading to pancytopenia.
